Summary:<\/strong><\/p>\n

Under the direction of the Marketing Director, the Community Outreach Coordinator is responsible for building relationships, identifying client specifications, promote company programs and services, raise awareness about company\u2019s vision and mission and monitor all community outreach activities with the goal of fostering positive community engagement and impact. This position reports to the management team regarding development and implementation initiatives that will increase company visibility within surrounding communities.<\/p>\n

Nature & Scope-Principal Areas of Responsibilities:<\/strong><\/div>\n

\n
\n(This list of duties and responsibilities is not all inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time)–<\/span>Conduct community needs assessments to identify key issues and areas where the organization can provide <\/span>support.<\/span>–<\/span>Establish and maintain relationships with referral sources, including hospitals, clinics, and other healthcare <\/span>providers, to generate client leads.<\/span>–<\/span>Develop and execute outreach plans to promote the agency’s services to potential clients, community <\/span>organizations, and healthcare professionals.<\/span>-Conduct presentations and informational sessions to educate the community and potential clients about the<\/span>agency’s home care services and benefits.<\/span>–<\/span>Monitor and analyze outreach efforts, track leads, and provide regular reports on outreach activities and their <\/span>impact on client acquisition.<\/span>–<\/span>Organize and participate in community events, health fairs, and other promotional activities to increase <\/span>awareness of the agency’s offerings.<\/span>–<\/span>Coordinate with the intake and scheduling departments to ensure a smooth transition for new clients and <\/span>facilitate effective communication between clients and the agency.<\/span>-Coordinates daily marketing rounds throughout the community and surrounding areas. Distributing marketing<\/span>materials to increase exposure of services offered.<\/span><\/p>\n<\/div>\n<\/div>\n

Knowledge\/Special Skills\/Abilities Needed:<\/strong><\/p>\n<\/div>\n

\n

-Excellent communication and interpersonal skills to build rapport with diverse community members.<\/p>\n

-Creative thinking to develop engaging outreach strategies and materials.<\/p>\n

-Understanding of community dynamics and the ability to identify key stakeholders.<\/p>\n

-Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.<\/span><\/p>\n

–<\/span>Ability to develop, plan, and implement short- and long-range goals is required.<\/span><\/p>\n

–<\/span>Computer literacy including MS Office 365, Google Suite, and Electronic Health Record systems.<\/span><\/p>\n

–<\/span>Experience in public speaking and presentations.<\/span><\/p>\n<\/div>\n

Job Description-Scheduling-8\/2021<\/p>\n

-Valid State of Wisconsin Driver\u2019s License and reliable insured vehicle to be able and willing to travel is required.<\/p>\n

-Ability to read, write and speak English and Spanish is preferred.<\/p>\n

Physical Demands:<\/strong> 80% sitting; 20% walking\/standing; on occasion the incumbent may be required to stoop, bend, or reach above the shoulders. The incumbent must occasionally lift and\/or move up to 25 pounds. Specific vision abilities required by this position include Close vision, distance vision and ability to adjust focus.<\/p>\n
